The Georgia Health Policy Center (GHPC) fosters rural and urban community health systems development through provision of technical assistance, strategic planning, sustainability planning, and community health needs assessments. Some of the areas addressed through these efforts include quality improvement, policy for environmental and systems change, benefits counseling, and care coordination to achieve health and health system improvement.
To accomplish the ultimate goal of population health improvement GHPC employs customized tools including the Sustainability Framework, formative assessments, systems mapping , and community health needs assessments.
The Community Health Systems Development team is a large group within the center. The team provides a logical progression of activities to support communities in accomplishing their goals by focusing first on developing a strategic mindset, then building requisite skills and capacities, and integrating sustainability strategies during implementation of community-developed health initiatives. To date the center has provided tailored technical assistance to more than 1,000 communities throughout Georgia and across the nation.
