Aligning in Action: The Health Project
Mercy Health uses community benefit dollars to fund many community-facing initiatives through the Health Project. Central to the Health Project’s direct services is care coordination and referral for social needs through the use of the Pathways Community HUB model, an evidence-based, pay-for outcomes model for community care coordination.
