Community Resilience for COVID-19 and Beyond: Leveraging a System for Health Care and Social Services Integration
Through analysis of case management processes that support patients’ health and social needs, researchers will identify how specific resources enabled by the aligned systems are being used to reduce COVID-19–related risks, address barriers to sheltering in place, and overcome social and economic disruptions of virus-related closures for vulnerable patients living in Contra Costa County, Calif.
