Screen and Intervene: Evaluation Plan
The purpose of this evaluation is to understand the effectiveness and efficiency of the Screen and Intervene: Connecting Food Insecure Patients to Resources program. This study is funded by the Florida Blue Foundation in an effort to address food insecurity using a collaborative, multidisciplinary approach.
