Rapid-Cycle Research Findings from the Population Health Innovation Lab at the Public Health Institute
Accountable Communities of/for Health (ACHs) are a community-driven model that supports resource alignment and fosters systems change activities to create the conditions for whole-person health and wellness. This study used data gathered from ACHs in California and Washington to understand when and why aligning produces positive outcomes.
