Evaluating Trinity Health’s Diabetes Prevention Program Network Hub
Trinity Health launched its Strategic Health Approach to Promote Health Equity and Diabetes Prevention (SHAPE-DP) program to advance health equity and reduce health disparities by implementing a bold strategy that promotes optimal health for people experiencing poverty and other vulnerabilities, including unmet social needs. These needs will be addressed through a national network of collaborators (Hub). This hub includes clinical provider network and committed national partners across Trinity’s 16-state service area, representing over 16 million adult community members. The Hub will significantly increase enrollment in diabetes prevention programming and address unmet social needs in three priority populations (African American/Black and Hispanic/Latino individuals, and adults 65 years and older). Additionally, Hub partners will expand coverage by testing new types of coverage and payment risk sharing models to remove barriers to participation in lifestyle change interventions with committed payers.
Trinity Health engaged the Georgia Health Policy Center to perform a mixed methods evaluation of SHAPE-DP. The evaluation will integrate and analyze multisource data, including standardized programmatic data and electronic health record data, to examine program outcomes and inform process improvement. Longitudinal data from individuals enrolled in the program will also be analyzed. Key performance indicators include enrollment, retention, health outcomes, social needs screenings, and interventions. Quarterly reporting will inform real-time performance improvement opportunities for partners.
