- Facilitate the development and collective action of an external, cross-sector planning task force to guide the process, support data sensemaking, and make recommendations about priorities
- Support the collection, analysis, sensemaking, and reporting of secondary data/information (population-level health factors, determinants and outcomes; built and natural environmental hazards; and public health infrastructure) with a focus on disparities and inequities, and additionally capture an inventory of deployable assets and available resources)
- Support the collection, analysis, sensemaking, and reporting of primary data through statewide (urban and rural), key informant interviews, focus groups, and community conversations to understand the perception of health status, assets and challenges as well as gather recommendations for strategic action.
In support of efforts to update the state health improvement plan, GHPC will
- Crosswalk accreditation requirements and findings from the state health assessment
- Facilitate the development an internal strategic leadership team and an external planning task force to ensure that the state health improvement plan development and implementation is community-driven
- Rank public health policy and program priorities for the state by analyzing the state health assessment plan data and conducting a survey of key stakeholders and partners
- Develop an implementation plan (including a timeframe) for each priority issue, and help organize DPH staff and partners to implement the identified action steps
- Establish a process that will be used to track the progress on plan strategies and activities
