In partnership with the Georgia Department of Human Services’ Division of Aging Services (DAS), local Area Agencies on Aging (AAAs) facilitate and deliver Georgia HealthMatters health promotion and disease prevention programs in their region. These community-based programs promote healthy aging by focusing on chronic disease management and healthy living related to nutrition, physical activity, and lifestyle management. Programs are evidence-based, individually focused, and deliver proven results.
The Georgia Health Policy Center, in partnership with DAS and local AAAs, developed a comprehensive toolkit of tips, tools, and templates for DAS, local AAAs, and other partners to communicate about and market Georgia HealthMatters programs in a coordinated, statewide manner, with the goal of growing participation among older adults and referrals from providers.
