Health Action Planning for Affordable Housing
Health Action Plans, developed by Enterprise Community Partners, offer affordable housing developers a process to integrate health into the design and development or rehabilitation of their housing based on the most significant health needs of their residents. Through a combination of data analysis and community engagement, affordable housing developers work with public health professionals to prioritize and develop actionable strategies to address health needs of current and future residents.
In partnership with Enterprise, the Georgia Health Policy Center has developed several Health Action Plans for preservation and rehabilitation of several multifamily affordable housing projects. The process involves:
- Community health data is collected and analyzed. Key to generating locally relevant priorities for action, a rapid baseline health assessment can include local community health needs assessments, other national and local public health datasets, and understanding of the evidence-based interventions.
- Community engagement ensures that prioritized health needs reflect the issues identified by the resident population. Results of engagement are integrated with the findings from the data collection and analysis.
- Evidence-informed design and intervention strategies are identified that can address the prioritized health needs of the community.
- The developer selects which strategies will be implemented for each project and identifies how they will be implemented, which is formalized in the final health action plan.
- A monitoring plan is designed and an evaluation is implemented to assess the impact of the health action plan.
