Trinity Health is one of the largest multi-institutional Catholic health care delivery systems in the nation, serving more than 30 million people across 22 states. Trinity Health’s Transforming Communities Initiative (TCI) is an innovative funding and technical assistance initiative that couples advancing policy, system and environmental change with addressing social needs to make progress towards health equity in Trinity Health communities.

For 2022-2026, TCI is funding 10 hospital-community partnerships to build community capacity and accelerate community health improvement by implementing evidence-based strategies that directly impact needs, as identified by hospitals’ community health needs assessment.

The Georgia Health Policy Center serves as the TCI national evaluator and in this capacity is both designing a cross-site national evaluation strategy and supporting local evaluations in the 10 TCI-funded communities.
