Transforming Communities is an initiative of Trinity Health, one of the largest multi-institutional Catholic health care delivery systems in the nation. The five-year, $80 million initiative consists of grants, loans, technical support, community-matching funds, and other activities aimed at improving community health and well-being. The initiative targets three principal areas —obesity (especially childhood obesity), tobacco-free living, and social determinants —through use of evidence-based policy, systems, and environmental change approaches.
Georgia Health Policy Center (GHPC) is conducting a cross-site assessment intended to inform the design of the Transforming Communities initiative and measure its full impact over the grant period and beyond. The purpose of evaluating this initiative is to assist Trinity Health in strategically reviewing the combination(s) of resources that are used in the different sites; their collective influence on affecting progress towards policy, systems, and environmental changes; and the coalition and community factors influencing progress and successful adoption or implementation. GHPC is also supporting individual Transforming Communities sites to develop local evaluation plans.
