The Assessment for Advancing Community Transformation (AACT)
The Assessment for Advancing Community Transformation (AACT) tool allows communities to directly collect and use data about their collaborative efforts transform communities. By assessing six key themes and 22 subitems, the tool provides data to community collaboratives in order to reflect, prioritize, and align actions based on their shared understanding where they are now and where they want to go.
With support from the Robert Wood Johnson Foundation, the tool was developed by 100 Million Healthier Lives/Institute for Healthcare Improvement, County Health Rankings & Roadmaps, and Georgia Health Policy Center. Together, representatives from each organization with practice-based knowledge and experience providing technical assistance, reviewed a convenience sample of research and reports related to multisector partnerships and organizational readiness. The key themes that emerged were used to support the conceptual foundation for the AACT design.
Unlike other assessments, AACT is community-driven and -directed and no outside support is needed to use the tool or interpret results. Much of the value of AACT comes from the group discussion with those who completed it. Understanding the various perspectives will provide important insights into the collaborative’s work.
